Penn Treaty Long Term Care Claims
To submit a Penn Treaty Long-Term Care Insurance claim, call them directly at their policy holder services department. One of their representatives will send the claimant an information packet containing instructions and required forms they will need to complete and either mail or fax back to Penn Treaty. The forms can also download and printed on their website.
