Long Term Care Insurance

To file a long term care insurance claim with Bankers, the application for benefits packet (claims form) will need to be completed, which will include the information need from the claimant, their doctor, and the claimants facility or provider. Below are the instructions on how to complete the forms.
Forms for the claimant will need to complete - Forms A & B, will need to be completed in full and either the claimant or their representative will need to sign. If their representative completes and signs the form, please attach a copy of the appointment.
Forms the claimants doctor will need to complete - Form C, the claimants doctor will need to complete in full and sign.
Forms the claimants facility will need to complete - Form D, the facility (nursing home, assisted living, etc.) will need to complete this form in full. Which the facility will also be asked to provide supporting documents with the application. These are highlighted on the form. If you are receiving home health care benefits, Form E will need to be completed by the provider. Supporting documents will also be required.
Once the claim has been approved, Bankers requires monthly bills from the facility with the policy number shown. Or if home health care benefits are being received, Bankers requires regular submission of bills and the provider daily care sheets showing the policy number.
